CAMP AGREEMENT & WAIVER

By paying my deposit, I hereby acknowledge that I/my child am/are voluntarily participating in an Irish Dancing camp offered by the Cox Academy of Irish Dance, located at Big 4, Lakeside in Narrabeen from 18-20 September.

I understand that dance fitness activities involve physical exertion, which may be strenuous and may cause physical injury. I acknowledge that it is my responsibility to consult with a GP prior to and regarding my participation in Irish Dancing if I have any concerns. I represent and warrant that I am physically fit and I have no medical condition that would prevent my full participation in the dance camp.

I understand that it is my responsibility to inform the instructor of any pre-existing medical conditions, injuries, or physical limitations that may affect my ability to participate safely in the dance fitness classes. I understand that the instructor may modify or adapt the exercises to accommodate such conditions, injuries, or limitations, but that it is ultimately my responsibility to monitor my own participation and to cease any activity that causes discomfort, pain, or exceeds my personal limitations.

In consideration of being permitted to participate in Irish Dancing, I hereby release and waive not to sue The Cox Academy of Irish Dance, its owners, instructors, employees, and agents from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, while participating in Irish Dancing, whether caused by the negligence of the releases or otherwise.

I understand and acknowledge that the Cox Academy of Irish Dance does not provide insurance coverage for participants, and I am solely responsible for obtaining my own insurance coverage. I have read this waiver and release of liability in its entirety and fully understand its terms. I understand that by participating in these classes, I am giving up legal rights and/or remedies that may be available to me. I voluntarily agree to be bound by its terms.
